If this form is not completed and submitted, you will not be certified for VA Benefits this semester.

VA BENEFIT PAYMENT AGREEMENT

Students must be enrolled and attending classes at NTC to receive payment of veteran educational benefits.

Short term and/or accelerated classes could have a significant impact on veteran educational benefits.

Only program required courses which have not been successfully completed for graduation purposes can be certified for Federal Veteran Educational Benefits.

Students are responsible for notifying the NTC Veteran Certifying Official whenever they drop/add classes. If those changes are not reported in time, the student may be required to refund the over payment to the VA.

NTC is required to return CH33 tuition payments for students who withdraw or no show for courses resulting in a balance due to NTC for the student.

Students must meet and maintain satisfactory academic guidelines established by NTC to continue receiving veteran benefits.

Students who have applications pending approval from Chapter 33 and/or WI GI are required to pay for tuition/fees by the established due dates. You will be reimbursed if and when you are approved or when VA payments are received.

The WI GI bill tuition remission programs only covers program and material fees. Other fees must be paid by the students by established due dates. Students can contact the Veteran Certifying Official to get the balance in fees not covered by the WI GI Bill.

By submitting this form, I:

Give my permission to the Veteran Certifying Official to release educational information and information necessary for my veteran benefits.

Understand that I am responsible for paying fees and tuition costs within the deadlines established by the College.

Understand that I am responsible for informing the Veteran Certifying Official of any changes in my enrollment.

Agree to pay any tuition and fees that the VA does not pay.

Have read and agree to the VA Benefit Payment Agreement guidelines and will contact the Veteran Certifying Official with questions.
